"Episode 209" - Follow the journeys of Jeff, a 43-year-old factory worker from Kingsport, Tennessee, and Autumn, a 30-year-old fifth grade teacher from Santa Clarita, California, from their "before" phases, then as they undergo their various procedures and, finally, when the end results are revealed to their families and friends. Jeff and Autumn were given a truly Cinderella-like experience, as they were able to change their looks in an effort to transform their lives and destinies and to make their dreams come true. This was all accomplished through the skills of an "Extreme Team," including plastic surgeons, eye surgeons and cosmetic dentists, with a talented team of hair and makeup artists, stylists and personal trainers.

Jeff, a 43-year-old factory worker from Kingsport, Tennessee, struggled all his life with obesity and made a life-saving vow to change his destiny after he shattered a plastic chair when sitting on it. Before Jeff applied to "Extreme Makeover," he'd already lost 200 pounds on his own in only one year with a self-created rigid diet and exercise regime. In order to remove the large amounts of excess skin that remain on his frame, Jeff will have to undergo an extended body tuck, a high risk procedure which yields dramatic results for candidates who can only pass rigorous medical tests. His story will be told over two can't-miss episodes ("209" and "210"), with the first depicting Jeff's facial surgery and his will and determination to be a candidate for the body tuck, despite the life threatening risks he faces due to years of obesity. The second will feature the dramatic conclusion.

Autumn, a 30-year-old fifth grade teacher from Santa Clarita, California, suffers painful memories of childhood, when she was always considered less attractive than her three sisters. Autumn speaks candidly about how her mother's criticism of her looks affected her self-esteem. She undergoes intense facial and body surgery and endures an emotional journey to accept her new beauty. She also works with trainer Michael Thurmond, who sculpts and transforms er body with his intense workout regimen. Both her plastic surgeon, Dr. Perlman, and Michael Thurmond claim this is one of their BEST transformations ever and both attend her final revealing. Autumn had septo-rhinoplasty, upper and lower blepharoplasty (eyelift), chin-jowel implant, liposuction under chin, excision of Buccal fat pads, brow lift, fat injections in upper lip, smooth beam treatment, facial, teeth straightening, 6 upper veneers and Zoom whitening.

In a fast and furious wardrobe/hair/makeup overhaul, viewers will see his amazing before and after results in the first half-hour of the show. And Sam dispenses his tricks and tips in the fields of fashion, personal style and grooming with "news you can use" to give yourself a whole new look without any plastic surgery in his "Makeover in a Minute" countdown.
